Three hazards a manufacturing shift typically covers — lockout/tagout on press changeover, solvent handling at the cleaning station, and a guarded line restart after maintenance. Each row is the brief the shift lead runs at the line-side briefing, then has the crew sign against.
Hazards
- Switchover from blank-and-form to draw set, dies not yet verified zero-energy
- Two contractors on the floor today, unfamiliar with the new energy-isolation points
- Lube-line residual pressure not yet bled from yesterday’s run
Controls
- Supervisor-signed zero-energy verification before any die access
- Tag-out reviewed at the energy-control point — not just the press main
- Bleed-and-verify on the lube loop with a gauge tap check before die work
PPE
- Cut-resistant gloves during the die change
- Safety glasses with side shields
- Hearing protection during the verification cycle
Hazards
- Shift changeover means a fresh day-crew on the parts washer
- Ventilation fan service ticket open since yesterday
- New totes of an isocyanate-bearing primer staged within 3 m of the open cleaning vat
Controls
- SDS re-read at the start of shift with the day-crew
- Bump-test the LEL meter before opening the vat lid
- Re-route the primer totes to the flammables cabinet before the washer is back online
PPE
- Half-face with organic-vapor cartridges (cartridge change-out logged)
- Splash goggles over the glasses, nitrile gloves under chemical gauntlets
- FR coveralls because the vat sits 4 m from the welding bench
Hazards
- Conveyors and the indexer back online after a four-hour maintenance window
- Guarding panels re-hung but interlock circuit not yet continuity-tested
- Pinch point at the transfer arm where a near-miss was filed two weeks ago
Controls
- Barrier-test every guard interlock at the panel before energizing
- Slow-jog the indexer from the teach pendant with a spotter at the pinch point
- Restart authority held by the lead until the first 5 cycles clear without a stop
PPE
- Cut-A gloves near the transfer arm
- Steel-toe boots, hi-viz Class-2 vest during the slow-jog window
- Hearing protection in the equipment envelope (NRR ≥ 25)
